Services that are delivered in a wide range of settings such as homes, community clinics, community centres and schools. They commonly include adult community nursing, specialist long-term condition nursing, therapy services, preventative services such as sexual health and smoking cessation clinics, and child health services including health visiting and school nursing.

These services aim to keep people well, treat and manage diseases and support people to live independently in their own homes. This can also include voluntary organisations. Community Services are divided into two areas:

1. Community Care Services: Social care services that can help you live a full, independent life and to remain in your own home for as long as possible.

2. Community Health Services: Health Services that are provided outside hospitals, such as district nursing.
